Coordination Support

As Coordination Support specialists, we will assist you to understand your NDIS plan. We do this by:-

  • Taking referrals and assessing your support needs, turning them into action plans;

  • Understanding your individual needs and providing an interpretation of your NDIS support plan

  • Incorporating the funded supports into your unique plan;

  • Organizing the necessary assessments to identify the type and amount of funding required;

  • Assisting you with the budget calculation and letting you know the breakdown of funds for each support type;

  • Consult with plan managers to establish the appropriate claim categories and allocate the appropriate funds;

  • Assisting you with budget monitoring in accordance with the program guidelines;

  • Researching, assessing, and sourcing several mainstream, community, and informal provider options that meet your needs and presenting them in an easy-to-read format. Connecting you with community, mainstream, and other government services; Negotiating on your behalf support services and prices;

  • Coordinate day-to-day activities; Communicate with key stakeholders internally and externally;

  • Management of your support service schedules, including booking appointments, appointment confirmation, and short notice placements and cancellations;

  • Work with you to prepare for periodic plan reviews (e.g. did we achieve the goals outlined? Did we obtain services that were of 'good value'?)

Assist Life Stage Transition

We will assist you in life stage transitions:

  • Supporting coordination and connection

  • Supporting tenancies and accommodations

  • Mentoring and peer support

  • Developing individual skills

  • Assisting with decision making and planning

Assist Personal activities

If you are an NDIS participant who has been approved for the Core Supports category, you can access these services through your NDIS plan.

Our team will assist and supervise your daily activities to enable you to live your life with independence and ease.

We can assist with a range of everyday activities including, but not limited to:

  • Showering, bathroom assistance, mealtime support

  • Attending events, appointments, or simply planning your day

  • Going to the toilet & Personal hygiene

  • Getting in and out of bed

  • Feeding in conjunction with a support plan

  • Moving around the home

  • Mobility and hoist transfer

  • Dressing & Grooming

Assist household tasks

We can support you to maintain your independence within your home by providing the following services:

We can assist with a range of household tasks including:

  • assisting with cleaning, meal preparation, ironing, laundry, yard work and maintenance, and a range of other services to meet individual client's needs

  • Any general household chores that you may find challenging

Assist Community Access / Participate Community

Our caring and experienced staff at Crystal Disability Support provide an array of community services to NDIS participants, including, but not limited to:

We can assist community access by:

  • Attending outdoor and social activities and events including concerts, movies, sporting events, shopping, school holiday programs, art group, day trips

  • Involvement in swimming, bowling, and other low-impact sport

  • Learning how to use public transport or how to drive
